Acne Scar and Large Pore Treatment With Microneedle Fractional RF

Acne scars and large pore size are common and very frustrating skin problems affecting millions of Americans of all skin types. The appearance of scarring and large pores can affect self esteem and confidence in teens and adults alike. In our Southlake and Dallas offices we have been successfully treating patients with acne scars and large pores with a wide range of combination cinical treatments such as series of deep microdermabrasion, micropeels, TCA peels, laser skin resurfacing and pore refining skincare. Each of the above procedures leads to improvement in mild to moderate and relatively shallow scarring. Pore size reduction needs continued use of pore refining skincare products and mainatenance clinical treatments.

Our search for better and safer treatments for all skin types and colors has led to adding our newest treatment option for acne scars and large pores, the 3D Dermalift MFR (Microneedle Fractional Radiofrequency) for scar and wrinkle treatment, think of this treatment as Thermage rf delivered with microneedles for vastly improved results. So far, in the last 12 weeks of having this system, we have treated over a dozen patients of all skin colors and types and have had excellent results and patient satisfaction, with NO complications.

With any new treatment, I like to look at clinical data from various sources, other than manufacturer of the treatment equipment, for an objective analysis of safety and efficacy of the treatment before adding it to our practice.

For the 3D Dermalift MFR, I reviewed several clinical stidies worldwide comparing different types of radiofrequency, (monopolar vs bipolar, fractional vs traditionsl etc.), different types of delivery methods (micronedle vs non-invasive), depth of needles (0.5 to 3mm), types of needles (we have gold plated and insulated microneedles) to determine the best system for our practice. Our 3D Dermalift is a custom manufactured system capable of delivering all the possible rf options with computerized precision needle depth control for different skin needs.

We use the 3D Dermalift MFR for wrinkles, scars, stretch marks, skin tightening, necklift, facelift, cellulite reduction and pore size reduction. We are evaluating the efficacy of this treatment for excessive underarm sweating and it looks promising.

Fractional technology overcomes the problems of ablative lasers, such as inaccurate depth control and damage to the epidermis. Minimally invasive fractional radiofrequency microneedle devices allow for more-selective heating of the dermis.
OBJECTIVE:

To evaluate the clinical efficacy of fractional radiofrequency microneedle (ERM) treatment in acne scars and large facial pores.
MATERIALS AND METHODS:

Thirty patients with acne scars and large facial pores were enrolled. Bipolar radiofrequency energy was delivered to the skin through the electrodes of the FRM device. Skin lesions were evaluated according to grade of acne scars, Investigator Global Assessment of large pores, skin surface roughness, transepidermal water loss (TEWL), dermal density, microscopic and composite image, sebum measurement, and questionnaires regarding patient satisfaction.
RESULTS:

The grade of acne scars and Investigator Global Assessment of large pores improved in more than 70% of all patients. Skin surface roughness, dermal density, and microscopic and composite images also improved, whereas TEWL and sebum measurement did not change.
CONCLUSION:

Clinical improvement from FRM treatment appeared to be related to dermal matrix regeneration. FRM treatment may be effective in improving acne scars and facial pores.


Some studies show that Monopolar fractional rf works better than bipolar fractional rf due to ist abiity to reach deeper in dermis. Our 3D dermalift MFR system is capable of doing bipolar or monopolar treatments and I persoanlly favor monopolar over bipolar for deep wrinkles, acne scars, large pores and face or necklif due to its ability to generate collagen in deeper dermis than bipolar.

The biggest adavantage of this treatment is the ZERO DOWNTIME and minimal discomfort compared with invasive or ablative lasers.
